 Those who want Pilly’s pills are people who have stronger desires than average people. 
Pilly uses those lustful desires and pills as its tools to distinguish humans who are in need of him, and offers them his pills.
“If you take this pill, you’ll be only concentrating on your work of desire until the goal is achieved.
This means, you won’t be taking rest, consuming food, nor getting sleep at all. A small side effect is
sweating a bit, but since it ain’t addictive, you can always stop whenever you start to sweat”, 
When Pilly is done explaining the effect of the pill, he asks the person whether he or she is willing to
